In a saucepan, combine the milk, cream, and sugar over medium heat. Stir continuously until the sugar is completely dissolved. (3-5 minutes)

In a separate bowl, whisk together the egg yolks until they become light yellow (approximately 4 minutes). Gradually pour 1/2 cup of the warm milk mixture into the egg yolks, whisking constantly to prevent scrambling. Return the egg yolk mixture to the saucepan, stirring continuously, and cook until the custard thickens enough to coat the back of a metal spoon (about 8-10 minutes). Remove from heat.

Stir in the chocolate hazelnut spread, espresso powder, and vanilla extract until fully incorporated. Strain the mixture through a fine-mesh sieve into a clean bowl. Cover and refrigerate until thoroughly chilled (about 3 hours).

Pour the chilled custard into your ice cream maker and churn according to the manufacturer's instructions. Once churned, transfer the gelato to an airtight container and freeze until it reaches your desired consistency.

For a more intense hazelnut flavor, lightly toast the hazelnuts before blending them into your chocolate hazelnut spread.
If you don't have instant espresso powder, you can substitute with finely ground espresso beans.
Be careful not to overheat the custard, as this can cause the eggs to curdle. If the custard starts to look lumpy, remove it from the heat immediately and whisk vigorously.
