While the casserole is baking, prepare the cream sauce. In a saucepan over medium heat, melt the remaining 4 tablespoons of butter. Whisk in the flour and cook, stirring constantly, for about 1 minute until smooth. Gradually whisk in the milk until the mixture is smooth. Bring to a boil, then reduce heat to low and simmer, stirring occasionally, until thickened, about 3 minutes. Stir in the remaining 1/2 teaspoon salt, 1/4 teaspoon pepper, and nutmeg. Cook for about 8 minutes on low heat, stirring occasionally, then remove from heat. (15 minutes)
